you need to do -
1. Navigate
to the Application Form using the Menu on the left.
2. Complete
ALL fields of the form.
3. Check the
details you entered are correct.
4. Click on
the "Print this Page" button at the bottom of the
form.
5. Sign the
Application Form.
6. Make copies
of your 3 identification documents. See
Identification Requirements
for details
7.
Fax the signed application PLUS the copies of three pieces of ID to us at 02 9326 3288 .
8.
If you don't have or don't want to use a credit card, you can post the signed application form along with copies of your identification to us with a cheque or money order to PO Box 176 Randwick NSW 2031.
How much does it cost?
Our fee is $100.00 plus GST of $10.00 together with the appropriate application fee by the State or Territory Registry.
In New South Wales the Registry Fee is $36.00 resulting in a total cost to you of $146.00.

When will I receive
my certificate?
For New South Wales, an official certified copy of the Birth Certificate
you ordered will be posted to you, by us, within approximately
7 to 10 working days (or 24 hours for Urgent), following receipt of your application,
identification and payment.
Certificates from other State and Territory Registries
will be forwarded directly to you from the particular State
or Territory Registry and generally takes several weeks longer.
Why would delivery
of my certificate be delayed?
1. You
forgot to include payment.
2.
You did not provide 3 pieces of ID in accordance with
the requirements.
3. Written authority
was not provided for an application relating to a third party
