| Identification (ID) needs to
be copied and faxed or posted with the signed certificate
application form prior to the issue of any certificate. You
must provide one (1) form of ID from EACH
of the categories shown below.
For VICTORIA & ACT applicants – see IMPORTANT NOTE at the end of this page.
PLEASE
NOTE: If you are
unable to provide ID from Category A, you MUST
provide two (2) forms of ID from Category B AND
one (1) form of ID from Category C.
YOU MUST SUPPLY THREE FORMS OF IDENTIFICATION IN TOTAL.
